First Time Ordering Instructions

Print out the above three (3) forms.

Phone K-tel to find out what the prices are for the medications you wish to order.

Write drug name and prices on your order form in the space provided, and complete the forms in their entirety.

Fax or mail a copy of your completed order forms together with your doctor’s written prescription to:

NOTE: If you have chosen to fax your order to us, we will need to receive your original prescription(s), written by your doctor, before your order can be shipped. Please mail the originoal prescription(s) to K-tel Meds as the above address.

YOU WILL NEED ADOBE ACROBAT READER software to view and print the forms. If you do not have Adobe Acrobat Reader software, you can get it here.

Re-Order Instructions

If you are re-ordering medications, all you need to do is complete the Prescription Medication Re-order forms.

If you are already a K-tel Meds customer, and your current prescription has no refills remaining, you will need to ask your doctor to write a new prescription. Your doctor can fax the new prescription to us. The toll-free fax number is 1-866-583-5300.
